TDC Requisition Form

Toronto Diagnostic Centre was established with the purpose of providing diagnostic services to residents in the east end of Toronto. We are focused on providing patient-centered care in a state-of-the-art modern facility with quick turnaround times for all reports. We work with a team of six radiologists to ensure reports are provided back to referring physicians/healthcare providers as soon as possible.

New Patients, IFH and Bluecross patients are welcome at our clinic. Wheelchair accessible.
